Privacy Complaints & Incidents



Patients, students, clients, and/or their legal representatives may file a Privacy Complaint at any time, if they feel their privacy rights have been violated.

File a Privacy Complaint by printing a Privacy Complaint Form and mailing or faxing it to the Privacy Office.


UF Workforce Members should report all known and suspected privacy incidents immediately!
Complete a Privacy Incident Report online and submit it via email, fax, or campus mail. Choose from two report forms:
