National Provider Identifier

Do I Need a National Provider Identifier (NPI)? If you are a health care provider (any type) and you bill for your services, the last deadline was May 23, 2007. Medicare and Medicaid have already started using the numbers. Payment for services will be discontinued if your NPI is not included in billing after May 23rd.

All health care providers who bill for services will need a personal NPI Number. If you have any questions, please call Caroel DeBose, NPI Coordinator for the Privacy Office, at 352-273-5092.

View the Instructions for applying for your personal National Provider Identifier number.
