UF Students and Access to the Shands HealthCare Information Systems
UF students who are employed or volunteering at UF or Shands HealthCare and need access to the Shands HIS for employment / volunteer purposes must apply directly to the Shands IT Department for that access. (Click on the link, then on Non-Shands Employee Access Request Form.) Student employees should not identify themselves as students on this form, but should indicate their status by role: volunteer, research assistant, clerk, etc.
UF Students who need access to the Shands HealthCare Information Systems for the purpose of completing course-work must have that access obtained for them by a sponsor from their college, using the policy and procedure below.

Students, sign the Shands Confidentiality and Security Agreement here. Use you UFID #, not your Social Security Number!
Sponsors complete the Student Data Access Application form. The form will accomodate multiple students and must be completed online. Follow the instructions on the form and in the procedure above. Incomplete forms will be returned unprocessed.
