Basic HIPAA Information

E-Mail Authorizations

  • Patients and research participants may email between caregivers and research coordinators only after patients/participants read and sign an Email Authorization (see our Forms page here).

HIPAA Joint Notice of Privacy Practices (NPP)

  • Provided to all patients (just once) and written acknowledgement obtained that the Joint NPP was given.
  • NPPs may be ordered in English or Spanish in bulk from Publication Services here.
